ACT@Scale to host final conference
Tuesday April 2nd, the EU project ACT@Scale is to host a final workshop in connection with the conference ICIC19 in San Sebastian in the Basque country, Spain.
The three-year EU project ACT@Scale has identified and widely distributed “best practices” within Care Coordination and Telehealth with the aim of reaching 100.000 citizens across Europe. CIMT’s contribution consists of a video consultation service for relatives to haematological patients, which in the course of the project has been put into practice at the Department of Haematology and the Department of Oncology at OUH. In addition, CIMT has been responsible for the communication efforts of the entire project.
The project, the results and the main messages will be presented at the International Conference on Integrated Care 2019 on April 2nd at 12.30 pm – 01.30 pm. To finish, an exciting panel discussion with experts within the field will be held, and the audicence will have the chance of asking questions and learn about the distribution of Care Coordination and Telehealth.
